To register for a new account please complete the following:
  1. Fill out the New Practitioner Application.
  2. After completing the online form, upload your Healthcare Practitioner License or Certification and then submit the uploaded copy with your registration. Or, you may email your scanned license or Certificate to: info@symbiopathic.com
  3. After verifying your registration, our accounts team will create an account for you accessible on this site. You will then receive an account invitation via email to create your password.

To register for a new account please complete the following:
  1. Patients with Healthcare Practitioner approval may establish an account to facilitate ordering and re-ordering. Fill out the Approved Ordering Form for your registration. 
  2. When your registration is approved, you will receive an email telling you how to log in and start ordering.
